Many of these grossing templates were made with generous help from Dr. Zarrin Hossein-Zadeh. Others are based on templates from institutions I’ve practiced at. For some, you’ll see that there are different versions/options. As with all templates, these must be customized for each case–no two cases are the same!
Lung wedge resection
Received Fresh for intraoperative consultation labeled ” ” is a [ x x cm] lung wedge specimen with a [ cm] staple margin. The staple line is removed and the underlying parenchyma is inked [ ]. The pleura is pink-tan, smooth and glistening [with/without puckering], and is inked [ ]. The cut surface possesses a [ x x ] cm white-tan, firm and ill-defined mass. The mass is [ ] cm from the pleura, and [ ] cm from the stapled margin. The remaining cut surface consists of pink-tan, spongy and grossly unremarkable parenchyma. Photographs are taken. A portion of the mass is submitted for frozen section. Representative tissue, including the remainder of the mass, is submitted in [ ] cassettes:
A1 = Frozen section remnant
A2-4 = remainder of the mass
A5-6 = tissue directly adjacent to the mass
A7 = closest stapled margin;
A8 = uninvolved parenchyma away from the mass.
Lung, lobectomy
Received [fresh/ in formalin] labeled ” ” is a [ x x ] cm intact pulmonary lobe with a visible bronchovascular margin. The pleuara is tan pink-red, smooth and glistening with a [ x x ] cm area of puckering located at [ ] from the bronchial margin. Pleura is inked [ ]; area of puckering is inked [ ]. The parenchyma shows a [ x x ] cm circumscribed and firm mass with a pink and gray, solid and lobulated cut surface. The lesion abuts the pleura but does not appear to involve a bronchus. The distance between the mass and the bronchial resection margin is [ ] cm. The uninvolved pulmonary parenchyma is congested and pigmented. Multiple/[ number] of hilar lymph nodes are present. The cut surface of the lymph nodes are tan-pink and anthracotic. Specimen photographs are obtained.

Thymus
Received [fresh/in formalin], labeled with the patient’s name “ ” and labeled “ ” is a [ x x ] cm [oriented/unoriented] fragment of tan-red and lobulated portion of thymus with a smooth and lobulated surface on one side and a [ x x ] cm portion of glistening smooth pericardium on the opposite side. On sectioning there is a [ x x ] mass located at [ ] from the margin of resection [ inked blue] and [ ] from the pericardium. The mass is [ill-defined/encapsulated/ well-circumscribed] and is [ tan/fleshy/solid/hemorrhagic/ lobulated/cystic] grossly; the remainder of the specimen is [tan/pink/red/necrotic/hemorrhagic/cystic]. The mass appears confined to the specimen/grossly involves the pericardium.
